Please note: This is an all-in-one form, so you do not need to complete a separate application for each additional pet or service animal. One-Time Registration: Upload government issued photo ID Only if you are not already pre-registered with customs. Pet travelers will be required to travel with the required documentation outlined on your permit/Conditions of Importation/Requirements Document. Include each additional pets TYPE (Dog/Cat/Companion bird/etc.; BREED (must not be on the restricted breed list; must indicate primary breed and if mix, indicate both the primary and secondary breed); AGE (Years, Months); WEIGHT (indicate unit, Example: 6.12 Pounds, etc.). You are not required to upload vaccine report or photo of the animal at this time. However, in doing so, you allow for us to scan to see if there are areas of concern that can delay or hinder your approval or travel. The required documents must be presented to Airline/Cruiseline and Customs upon time of travel.

REQUIRED (READ CAREFULLY TO AVOID DELAY): At the time of application, the Government ID (Passport or Driver’s License) with NIB Smart Card or Permanent Resident Card if resident of the Bahamas. This information will be used one time solely for the purpose of set up and viewed securely directly on our server at Bahamas Pet Passport. PRE ARRIVAL CHECK LIST (WITHIN 48 Hours Prior To All Bahamas arrivals with pets): The Bahamas Requires ALL pets and service animals be accompanied upon arrival with 1) the e-Permit printed or saved to your phone 2) an International Veterinary Certificate by government accredited veterinarian attesting the Conditions outlined by the Bahamas Government for the Importation of Cats and Dogs 3) vet-issued Vaccination/Health report for Dogs (Distemper, Hepatitis, Leptospirosis, Parvovirus and Adenovirus) or Cats (Feline Leukemia, Feline Rhinotracheitis, Feline Calicivirus, Feline Panleukopenia and Feline Pneumonitis) to be presented to customs. We strongly recommend taking along an additional copy of the paperwork as well. (for a total of two sets of documents)
