*RESTRICTED BREEDS: The Bahamas prohibits the following dogs and cats and their mix(es) from import/entry: DOGS: Pit Bulls, Presa Canario, Cane Corso, American Bulldog; Staffordshire Terrier. CATS: Savannah, Chausie and Bangal.

REQUIRED DETAILS FOR VET VACCINE REPORTS OR CERTIFICATES OF VACCINATIONS (PLEASE READ CAREFULLY)

Required For Permitting Purposes:*

Attach in an email or UPLOAD online at BahamasPetPassport.com/Reorder within the upload section of the online reorder form, the Pet's Vaccine Report or Certificate of Vaccinations. on Veterinary Clinic letterhead or stationery or veterinary clinic printout to include: Clinic details* (Name, address, and phone contact); Owner details* (name, address and phone contact), Pet details* [Name, Microchip Number, Age (minimum age six-months), Non-Restricted Breed (primary, and if mixed, include non-restricted secondary-breed), Sex; Color; Weight) and Required Vaccines* ("See Health Record Requirements"), and Vet Details* (vet name, license#, signature and or clinic stamp).

REQUIRED VACCINES:

BOTH DOGS & CATS Require RABIES* 1-yr or 3-yr vaccine protection.*

DOGS require current-unexpired vaccine protection against Distemper, Hepatitis, Leptospirosis, Parvovirus, & Adenovirus (Commonly known as DHLPP or DA2PPv + L, (with 1-yr. or 3-yr. duration of immunity noted) and Fecal Exam with negative results for internal parasites.

CATS require vaccine protection against Feline Leukemia, Feline Rhinotracheitis, Feline Calicivirus, Feline Panleukopenia and Feline Pneumonitis (commonly known as FVRCP-C+FeLV or FVRCP+Ch+FeLV (with 1-yr. or 3-yr. duration of immunity noted), and Fecal Exam with negative results for internal parasites. .

Pre-arrival from a Rabies-Free Country - Within 10 Days prior to Bahamas Arrival, The Veterinary Form within your permit pages, must be completed by a licensed vet, certifying the vaccines administered for protection.

IF the pet or service animal is being exported from a High-Risk-Rabies country or a country, region or prior to entering the Bahamas, then the Rabies vaccine must 1) not be performed less than 30 days for full immunity; 2) not be more than 10 months for one-year vaccine; and 3) not more than 34 months for three-year vaccine.

BAHAMAS ARRIVAL DETAILS

DATE OF BAHAMAS ARRIVAL: The date you and your pet(s) will arrive in the Bahamas (MM DD YYYY).

BAHAMAS ARRIVAL PORT: Which air or sea port NAME or CODE will your pet be arriving in The Bahamas or Family Island? (Example: Lynden Pindling Airport (NAS), Bimini World Resort Marina, etc.)

Type the departure date when your Bahamas stay ends and you depart the Bahamas. If you will be departing the Bahamas to return to or travel to the U.S., you will be required to complete a CDC Import Form prior to return or travel to visiting here: Visit https://www.cdc.gov/importation/dogs/dog-import-form-instructions.html to complete a CDC Import form to present to customs and the airline/cruiseline for US bound travel.

1- A single - one-time entry e-Permit for pet(s) traveling together on the same trip and under the same owner. A separate e-Permit is required for each different trip or for pets belonging to different owners.

2- Additional Trips: Trip Start Date and Port required to Travel date details are required to request each new trip for Bahamas entry of pet(s).

Unexpired annual Fecal Exam details with normal or negative results will be required.

REQUIRED VACCINES: Please upload only current unexpired vaccine report for pets. Rabies vaccine is required for ALL pets. If the pet has been in a country/area/zone that is not considered rabies-free, then the 1-year rabies vaccine must not be less 1-month and not more than 10 months, and the 3-year rabies vaccine must not be less than 1-month and not more than 34 months. The DA2PP or DHLPP - Distemper Combination vaccine is required to protect against distemper, hepatitis, parvovirus, adenovirus, and Leptospirosis vaccine are also required for dogs. The FVRCP+FeLV+CH vaccines for cats are required to protect against feline leukemia, feline rhinotracheitis, feline calicivirus, feline panleukopenia, feline pneumonitis and fecal exam 1-yr (negative results).
