*RESTRICTED BREEDS: The Bahamas prohibits the following dogs and cats and their mix(es) from import/entry: DOGS: Pit Bulls, Presa Canario, Cane Corso, American Bulldog; Staffordshire Terrier. CATS: Savannah, Chausie and Bangal.

REQUIRED DETAILS FOR VET VACCINE REPORTS (PLEASE READ CAREFULLY)

To assure your request is approved (not rejected), please be certain the uploaded vaccine or medical health report contains the following: from s licensed vet or veterinary clinic and includes the following on veterinary clinic letterhead or stationery: 1) Veterinary Clinic's name, address and phone #; 2) Owner's name, address and phone #; 3) Pet's details: name , breed, DOB or age., sex, microchip #, weight and unit of measurement (lbs. or kgs.), microchip #, and'; 4) Required Vaccines with the given date, manufacturer, Lot # and expiry date; 5) License Vet's Full Name (First and Last), License #, Signature and or Clinic Stamp.

REQUIRED VACCINES:

BOTH DOGS & CATS Require RABIES* 1-yr or 3-yr vaccine protection.*

DOGS require current-unexpired vaccine protection against Distemper, Hepatitis, Leptospirosis, Parvovirus, & Adenovirus (Commonly known as DHLPP or DA2PPv + L, (with 1-yr. or 3-yr. duration of immunity noted).

CATS require vaccine protection against Feline Leukemia, Feline Rhinotracheitis, Feline Calicivirus, Feline Panleukopenia and Feline Pneumonitis (commonly known as FVRCP-C+FeLV or FVRCP+Ch+FeLV (with 1-yr. or 3-yr. duration of immunity noted).

BOTH DOGS & CATS are required to have a Fecal Exam (fecal parasitic screening) performed annually, semi-annually, etc. The normal or negative results must be documented on the IHC for travel to the Bahamas.

Pre-arrival - An International Health Certificate (IHC) and Veterinarian vaccine-certification form must be completed by a licensed vet within 48 hours prior to Bahamas arrival for entry.

Has the pet or service animal been in a High-Risk-Rabies country within the past six (6) months?
Please be alerted that IF a pet has been in a high-risk-rabies country within the past six (6) months, then the Rabies vaccine must 1) not be performed less than 30 days for full immunity; 2) not be more than 10 months for one-year vaccine; and 3) not more than 34 months for three-year vaccine. In addition to an International Health Certificate being completed within 48 hours prior to Bahamas arrival by a licensed vet, IF the animal has been in a high-risk-rabies country within the past six (6) months, then the IHC must be endorsed by a government accredited veterinarian with accreditation details and stamp. For example, USDA (US) accredited vet or CFIA accredited vet (Canada), etc.).

TRANSPORT MODE: How will pet be brought into the Bahamas:
Air Travel (commercial or private) Sea Travel (cruise lines, ferry, private vessel, etc.)

Country of Origin: What country is/are the animal(s) being exported from prior to entering the Bahamas?

TRIP STARTING PORT- Enter the name or code of the initial departure airport or seaport of origin that your pet(s) journey will begin before arriving in the Bahamas. Example, Miami International Airport (MIA), Toronto (YYZ), etc.

TRIP START DATE: The date your journey to the Bahamas with your pet(s) begins. For flights, this is typically the day you depart your home country. For sea travel, use the date the vessel departs its port of origin—not the date you arrive in the Bahamas or leave it.. (MM DD YYYY)

BAHAMAS ARRIVAL DETAILS

DATE OF BAHAMAS ARRIVAL: The date you and your pet(s) will arrive in the Bahamas (MM DD YYYY).

BAHAMAS ARRIVAL PORT: Which air or sea port NAME or CODE will your pet be arriving in The Bahamas or Family Island? (Example: Lynden Pindling Airport (NAS), Bimini World Resort Marina, etc.)

Type the departure date when your Bahamas stay ends and you depart the Bahamas. If you will be departing the Bahamas to return to or travel to the U.S., you will be required to complete a CDC Import Form prior to return or travel to visiting here: Visit https://www.cdc.gov/importation/dogs/dog-import-form-instructions.html to complete a CDC Import form to present to customs and the airline/cruiseline for US bound travel.

If the pet has been in a country/area/zone considered as "High Risk", then the rabies vaccine must not be less 30 days, not more than 10 months for 1-year vaccine, and not more than 34 months for 3-year vaccine. The DA2PP or DHLPP Distemper Combination vaccine to protect against distemper, hepatitis, parvovirus, adenovirus, and Leptospirosis vaccine is required for dogs. The FVRCP+FeLV+CH vaccines for cats are required to protect against feline leukemia, feline rhinotracheitis, feline calicivirus, feline panleukopenia, feline pneumonitis.
