*RESTRICTED BREEDS: The Bahamas prohibits the following dogs and cats and their mix(es) or resemblances from import/entry: DOGS: Pit Bulls, Presa Canario, Cane Corso, American Bulldog; Staffordshire Terrier. CATS: Savannah, Chausie and Bangal.

REQUIRED DETAILS FOR VET VACCINE REPORTS OR CERTIFICATES OF VACCINATIONS (PLEASE READ CAREFULLY)

On Veterinary Clinic's letterhead , stationery or print-out. and must include the following:
Clinic details* - Name, address, and phone contact;
Owner details* - Name, address and phone contact;
Pet details* - Name, Age or Date of Birth(minimum age 6-months), non-restricted primary Breed, and if mixed, include the actual non-restricted secondary-breed, for example "Shihtzu-Poodle" or "Shi-poo"), Sex (Male/Female); Color; Weight) and Required Vaccines* ("See Health Record Requirements"); and
Vet Details* - Vaccine reports require the Full name of License# that administered the vaccines or otherwise attests that the vaccines were administered. For Certificates: In addition to licensed vet's full name, the vet's License #, signature and or clinic stamp are also required.

REQUIRED VACCINES:

BOTH DOGS & CATS Require RABIES* 1yr or 3yr vaccine protection.*

DOGS require:
Rabies 1yr or 3 yr + Given Date and Due Date, with Manufacture Name, Serial #/Lot #
DHLPP or DA2PPv (1yr or 3yr Combination Vaccine for Distemper, Hepatitis, Leptospirosis, Parvovirus, & Adenovirus
Leptospirosis (with 1-yr)

CATS require:
Rabies 1yr or 3 yr + Given Date and Due Date, with Manufacture Name, Serial #/Lot #
FVRCP (Combination Vaccine for Feline Leukemia, Feline Rhinotracheitis, Feline Calicivirus, Feline Panleukopenia and Feline Pneumonitis)
FeLV (with 1-yr. or 3-yr. duration of immunity noted), and Fecal Exam with negative results for internal parasites. .

PRE-ARRIVAL
Pet's traveling from a Rabies-Free Country - Within 10 Days prior to Bahamas Arrival, The Veterinary Form within your permit pages, must be completed by a licensed vet, certifying the vaccines administered for protection. We strongly recommend having your eet complete your documents in advance of planning. The 10 Day window should assist in the event there are vaccines or details that require updating by vet.

If the pet's country of origin or country is from a country that is High-Rabies-Risk, then the Rabies vaccine must not be less than 30 days, and must not be more than 10 months for 1-year vaccine and not more than 34 months for 3-year vaccine.

Country of Origin/Export: What country is/are the animal(s) being exported from prior to entering the Bahamas?

TRIP STARTING PORT- Enter the airport or seaport name or code your pet(s) journey will begin before arriving in the Bahamas. Example, Miami International Airport (MIA), Toronto (YYZ), etc.

TRIP START DATE: The date your journey to the Bahamas with your pet(s) begins. (MM DD YYYY)

BAHAMAS ARRIVAL DETAILS

EARLIEST DATE OF BAHAMAS ARRIVAL: If you don't have an exact date, give us the earliest date you and your pet(s) you're planning for your pet to arrive in the Bahamas (MM DD YYYY). Please Note: Your Single-Entry Permit is good for just one-entry from the earliest date you provide us and must be completed once before the 6-month expiry date of the permit.

BAHAMAS ARRIVAL PORT: Which air or sea port NAME or CODE will your pet be arriving in The Bahamas or Family Island? (Example: Lynden Pindling Airport (NAS), Bimini World Resort Marina, etc.). I the port changes at a later date, pemit is still good so long as the permit owner, pet name(s), country of origin, and actual travel date is on or after the earliest date of arrival you provided us and so long as completed before 6-month expiry of permit.

Type the departure date when your Bahamas stay ends and you depart the Bahamas. If you will be departing the Bahamas to return to or travel to the U.S., you will be required to complete a CDC Import Form prior to return or travel to visiting here: Visit https://www.cdc.gov/importation/dogs/dog-import-form-instructions.html to complete a CDC Import form to present to customs and the airline/cruiseline for US bound travel.

2- Additional Trips: Trip Start Date and Port required to Travel date details are required to request each new trip for Bahamas entry of pet(s).

Unexpired annual Fecal Exam details with normal or negative results will be required.

REQUIRED VACCINES: Please upload only current unexpired vaccine report for pets. Rabies vaccine is required for ALL pets. If the pet has been in a country/area/zone that is not considered rabies-free, then the 1-year rabies vaccine must not be less 1-month and not more than 10 months, and the 3-year rabies vaccine must not be less than 1-month and not more than 34 months. The DA2PP or DHLPP - Distemper Combination vaccine is required to protect against distemper, hepatitis, parvovirus, adenovirus, and Leptospirosis vaccine are also required for dogs. The FVRCP+FeLV+CH vaccines for cats are required to protect against feline leukemia, feline rhinotracheitis, feline calicivirus, feline panleukopenia, feline pneumonitis and fecal exam 1-yr (negative results).

Bahamas Pet Import Eligibility & Acknowledgment Pet travel to The Bahamas is subject to approval by the Government of The Bahamas and is not guaranteed. Certain dog breeds (including mixes or look-alikes)—Pit Bull–type dogs, American Staffordshire Terrier, Staffordshire Bull Terrier, American Bully, Presa Canario, Cane Corso, and Dogo Argentino—and certain cat breeds (including hybrids)—Savannah, Bengal, and Chausie—are restricted or prohibited. Breed eligibility is determined by validated veterinary health documents, appearance, and official assessment, not solely by owner belief or description; pets determined to be ineligible will not be approved. All information and documents submitted must be accurate and unaltered, and all fees are non-refundable. Where approval is granted, authorization is issued for one (1) trip only, for the specific pet and registered owner, permitting a single entry on or after the approved Bahamas arrival date and before the six-month expiration period calculated from that arrival date; circumstances beyond the traveler’s control do not extend permit validity, and authorizations may not be reused, transferred, or applied to a different pet, owner, or destination.
