*RESTRICTED BREEDS: The Bahamas prohibits the following dogs and cats and their mix(es) from import/entry: Pit Bulls, Presa Canario, Cane Corso, American Bulldog; Staffordshire Terrier; and Savannah, Chausie and Bangal. If the animal in the identifying photo presents as one of or a mix of the listed Restricted Breeds, additional proof may be required in order for a permit for import to be issued.

Please note: This is an all-in-one form, so you do not need to complete a separate application for each additional pet or service animal. Simply UPLOAD or EMAIL the required VACCINATION REPORT(S) for each pet you are applying for. The vaccination reports must include the following identifying details for pet's: Name; Age/Date of Birth; Color; Predominant BREED listed first, followed by specific mix(es) if any (Example: Shihtzu Poodle); Male/Female status; Neutered status; Microchip Brand & Number if available Each pet must also have a COLOR PHOTO.

REQUIRED (READ CAREFULLY TO AVOID DELAY): At the time of application, the health/vaccine report for all animals must indicate that 1) The RABIES vaccine is not less than 30 days old and unexpired; and 2) The DHLPP/DA2P vaccine (distemper combination vaccine for dogs) or FVRCP vaccine (for cats) must be current and unexpired. WITHIN 24-48 HOURS PRIOR TO BAHAMAS ARRIVAL, Proceed to licensed vet to 1) have an IMP-1B Form Completed and Signed 2) Have a licensed vet issue a health certificate for travel, indicating the pet is safe and cleared for travel; that the Rabies vaccine complies with requirements (not less than 30 days administered based on the date given by, last done, etc., not more than 10 months for a one-year vaccine; and not more than 34 months for a three-year vaccine); 2) that the DHLPP or FVRCP is current; and 3) that the animal is free from internal and external parasites. All vaccines must remain current and not expired throughout Bahamas travel. If documents are not received along with this application or if there is an issue with the documents we receive, our agent will contact the applicant with instructions to email the required documents. PRE ARRIVAL CHECK LIST (WITHIN 24-48 Hours Prior To All Bahamas arrivals with pets): The Bahamas Requires ALL pets and service animals be accompanied upon arrival with 1) Permit, 2) vet-signed IMP 1B Form and 3) vet-issued Vaccination/Health Certificate, to be presented to customs.
