- Upload online or email your vaccination Report or health documentation:
Vaccine Report page, health report, or invoice for vaccinations must indicate Rabies Vaccination is not less than 30 days at the time of application, and DHLPP (canines) or FVRCP (felines) must be current-unexpired. Any other supporting documentation as authorized officers require. The vaccination report must include a veterinarian or animal clinic contact details and signature or stamp; owner or handler details; and the Pet’s identifying details, such as Name, specific Primary Breed, both Primary and Secondary breeds (if “mixed”), Age or Date of Birth, Color, Weight, etc.
Color Photo
The authorizing officer of BDOA uses photos to visually determine whether the pet matches the animal described on both the application & vaccination report.
Via Online Submission: Once your online application has been completed, you will be prompted to “upload” your vaccination report and color photo. Once uploaded, submit, and your application will be reviewed by an agent for pertinent details needed for submissions to BDOA.

Completing the application and providing the required documentation alleviates issues in your application being submitted, approved, and delivered. If for any reason, you have not received a response within 30 minutes of your submission, please email permits@bahamaspetpassport.com or call us for free on WhatsApp at 1-242-449-9622.
